Some points:
  • A full ATX HTPC is not necessarily an eyesore. You just need the right case and it looks like stereo equipment
  • What are you going to be using it as--just watching shit or some gaming--and so, what level? If you're going to be playing the latest Doom a microATX or ITX might not cut it.
  • Running an HDMI-out that pipes both audio and video out is the easiest way to connect things. You'll have to be sure the board supports this, or you're in for a video card as well. Further, I remember running a MicroATX years ago, and it only supported stereo out over HDMI. Surround wasn't going to happen.

My build is a full ATX & Case, running a GEforce 760, older 6-core AMD processor, Blu-ray drive, this case: http://www.newegg.com/Product/Product.aspx?Item=9SIAA5W3RS8449, Running Win 10 on an SSD and a 1TB "data" spindle drive, 32GB RAM, and can play any games on it up to and including the newest Doom no problem.

For the case I'd recommend against it because I've had issues with the dive bay panel--however, Silverstone makes HTPC cases in the ATX format.

Now, if you're just wanting something to watch media, you could probably invest in something like a QNAP NAS device (I recently bought a bare 4-disk one and plopped four 3TB drives into it--and RAID-5'ed it). I got one recently because of the whole WMC/Win 10 fiasco since HDHR DVR will run on it, but it has HDMI out and comes with KODI, so virtually everything will play on it. The only thing I haven't tried is optical discs since I have an HTPC for that. However, if I wasn't running an HTPC as a gaming rig as well and wanted to slim things down, I would probably would just run the NAS as an HTPC since it will do an HDMI out and play just about anything. The only thing I'm unsure about is whether or not you can hook a USB BD-ROM to it and play disks on it. I'll have to try it out.
