Oh, buddy, has it ever! I'd say it's almost as synonymous as baseball and fisting [see St. Louis Cardinals]. Although to be fair, it's been mostly relegated to "European-style" hockey leagues; and even then, it's mostly just the French. Go figure.
Notable holdouts in the NHL include, and are limited to, the St. Louis Blues. If I recall correctly, tradition and personal preference mandates that they fellate one-another before, after and in-between shifts.

Actually, there should be more thread merging in the Bears forums. 13 threads on the same topic makes it an eyesore to even go into.
