While I am sure mutually willing participants comprise a part of the phenomena, it isn't a stretch to think that human trafficking compromises the majority of sexually exploited people. Again, the problem is NOT in the Middle East, but rather in the Americas and Asia.
You also seem to have a keen interest on the specific act of defecating on an individual with the goal of humiliation. This is called a paraphilic disorder. Where are paraphilic disorders the most prevalent. You guessed, right here in the good ole USA.

Nah...it's pretty clearly a paraphilic disorder according to the DSM V which says: "sexual masochism disorder is diagnosed in individuals who experience sexual arousal in response to extreme pain, humiliation, bondage, or torture. "
But you would know this as you are, apparently, a doctor. One who is keen to redefine sexual deviance, in a curious sort of way to be frank with you. I mean, you should know this.
